Women, Infants and Children (WIC) Information for Providers

Refer a Patient to WIC 

Providers could complete this WIC pediatricpregnancy, or postpartum referral form and ask your patient to call WIC at 408-792-5101 or text 888-413-2698.

Providers and other community agencies could send a request for the patient with this WIC Program Enrollment Request Form and fax it to 408-294-6315 or email [email protected] and WIC staff would reach out to the patient.

Refer a Patient for Breastfeeding Support

Complete the WIC referral forms above and fax it to 408-294-6315 or Email [email protected]

Ask your WIC participants to call the WIC Breastfeeding Support line at 408-794-0550.

Ask your non-WIC patients to call Nursing Mothers Counsel at (650) 327-6455.

    WIC recognizes and promotes breastfeeding as the optimal source of nutrition for infants. WIC encourages breastfeeding for one year and beyond. For women who do not fully breastfeed, WIC provides iron-fortified infant formula.  Enfamil Infant, Enfamil Gentlease, Enfamil ProSobee, Enfamil A.R and Enfamil Reguline are the current contract formulas available at WIC. WIC does not provide Enfamil NeuroPro, or Similac Pro-Advance.

    If one of the WIC standard contract formulas is not appropriate for your patient, therapeutic formulas are available with an appropriate medical diagnosis and are covered by Medi-Cal and health insurance. WIC is the payor of last resort.

    Here are suggested steps to obtain therapeutic formulas: 

    1. Refer the patient and submit medical justification to a pharmacy that accepts the patient's health plan. Once this request is approved, the pharmacy will provide the therapeutic formula to the patient.
    2. Fill out the WIC Pediatric Referral Form for the patient to submit to the WIC office. WIC requires a completed form for WIC participants receiving formula for medical conditions from any sources.
    3. Follow up with the pharmacy for approval of the health care provider's referral. It may take a few weeks to complete the approval process. While waiting for the approval, WIC may provide up to two months of the prescribed formula with a completed WIC Pediatric Referral Form.

    Should a WIC participant be denied the therapeutic formula by the health plan and with a copy of the denial letter, a local WIC office may provide the therapeutic formula. Therapeutic formulas available at the local WIC office include Similac Neosure, Enfamil Enfacare, Similac Alimentum, Enfamil Nutramigen with Enfloral LGG, Enfamil Nutramigen Concentrate, and PediaSure.

    For therapeutic formulas not available from the local WIC office, State WIC may review the referral with qualifying medical conditions in addition to documented intolerance to the contract (Enfamil) formula.

    A WIC Pediatric Referral Form must be completed by the health care provider every three months for a prescribed therapeutic formula, a formula change for the patient receiving formula from any sources, or for requesting a therapeutic formula from WIC.
